Your first visit to our office will involve a few special steps so that we can get to know you. Please read this entire page to help you make your first visit a pleasurable experience that helps you get all your questions answered and your problems resolved.

Patient Forms  

The following patient forms help us make sure we obtain important background information, like your medical history, current medications, and allergies, and obtain your insurance information. Save time on your first visit by printing out and completing the forms listed below before your appointment. At your convenience, take the time to read our financial policy and patient privacy notice. If at any time you have any questions about any of these forms, please feel free to call us or we will gladly help you when you arrive for your appointment. Be sure to bring these forms, your insurance cards and driver’s license with you on your first visit.

What To Expect

Being well prepared to your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you.  It also will help relieve any unnecessary anxiety you may be feeling. It would be helpful to bring any footwear that may be aggravating your condition, orthotics or arch supports that you may be using, and a list of questions that you’d like to remember to ask the doctor.

Bring Shoes

Always remember to bring different pairs of shoes, sneakers and slippers that you wear so we can be sure your shoes are not hurting your feet. We can measure your feet and the level of support in your shoes to be sure your shoes are helping you walk properly.
